Care Pathways

Six Ways We Serve Our Community

Whether you need a quiet listening ear, resources during a medical crisis, or peer connection, here is how we can help.

Individual Support

01. Emotional Care & Compassionate Listening

When walking through grief, diagnosis, or heavy life transitions, people often simply need a safe, non-judgmental presence. Our trained care volunteers provide one-on-one compassionate listening, offering a warm and steady companion without unsolicited advice or clinical formality.

How It Works: Held in quiet booked rooms at local Mississauga libraries, partner community spaces, or via phone/video. Scheduled at mutually convenient times.

02. Family & Caregiver Support

Caregivers carry an immense emotional, mental, and physical weight holding their families together. We provide specialized respite support for family members caring for aging parents, chronically ill spouses, or loved ones across Mississauga and Peel Region.

How It Works: Caregiver check-ins, structured respite conversations, and mutual care circles designed so the person holding everyone else has a safe space to be held.

03. Group Support & Gathering Circles

Facilitated small groups where community members share lived experiences, reduce loneliness, and cultivate meaningful friendships. Rather than facing hardship in isolation, participants discover they are not alone.

How It Works: Small gathering circles hosted monthly or bi-weekly in partner community rooms. Group size is kept intimate (6–10 people) to foster genuine safety and connection.

04. Practical Aid & Community Navigation

Healthcare systems, public benefits, community food banks, and elderly transit options can be overwhelming to navigate when you are in distress. Our team assists individuals in identifying verified community resources across Peel Region and connecting with the right agencies.

How It Works: Navigation assistance is available through direct inquiry via our support form or email. We help outline clear next steps and local contacts.

05. Community Collaboration & Agency Partnerships

No single non-profit can meet every human need alone. We actively collaborate with local churches, hospital chaplaincies, library branches, and non-profit health groups across Mississauga to coordinate care, share meeting facilities, and avoid duplication of services.

How It Works: Partner organizations can coordinate referrals or co-host support sessions by emailing info@newleafgracehub.ca.

06. Workshops & Creative Renewal Activities

Gentle, creative activities offer a powerful antidote to anxiety and isolation. We host community workshops featuring restorative arts, music appreciation, mindful reflection, and wellness discussions to help restore emotional equilibrium in a pressure-free environment.

How It Works: Periodic workshops announced through our community mailing list and website, hosted in partner library and community activity rooms.
